ご希望日時および必要事項をご記入のうえ、送信してください。内容を確認後、フォーム送信後3営業日以内に体験いただける日時を事務局よりご連絡いたします。（満席の場合にもご連絡はいたします。）

体験セッションは、各回少人数制・定員制のため、ご希望の日時に空きがない場合がございます。

その場合は、キャンセル待ち（ウェイティング）として自動的に受付させていただき、あわせて別日程の候補をご提案いたします。

This is a trial session booking request form for Miro Art Lab Azabudai Studio (for school-age children, ages 6+).

Please fill in your preferred date and the required information below,and submit the form.Our team will review your request and contact you accordingly.

All trial sessions are held in small groups with limited availability.If your preferred date is already fully booked,your request will automatically be placed on the waiting list,and we may suggest alternative dates.

In the event that the session is fully booked,our team will contact you by email within three business daysafter receiving your submission.

行動の特徴や、感覚の敏感さなど、活動時に配慮が必要な点がありましたら、差し支えない範囲でご記入ください。

なお、お子さまの状況によっては、安心して体験していただくために、保護者の方の同席をお願いする場合がございます。
